College Visit Form - This form is to be completed and signed by your parents, school counselor and the principal before going on your college visit, if you are missing school for the visit.  Then, once you have arrived at the college for the visit have an Admissions Office Representative sign for proof of the visit. This form must be returned within three (3) days to be marked as an excused absence.

Please complete the following items after you have completed and submitted your college application:

Recommendation Request Form - Use if you need a written HASD teacher/counselor/staff recommendation letter. Please allow the recommender at least 10 school days to complete their recommendation. (optional, depending on college)

Request a Transcript - Current Student - Use this link to tell the Counseling Office where and how you want your transcript sent. (Required)

APPLYING TO POST SECONDARY SCHOOLS/COLLEGES

 Step 1: Apply

Submit application on college website or the Common App.

The Common Application is a not-for-profit organization that serves students and member institutions by providing an admission application – online and in print – that students may submit to any of our 415 members.

Once completed online or in print, copies of the Application for Undergraduate Admission can be sent to any number of participating colleges. The same is true of the School Report, Midyear Report, Final Report and Teacher Evaluation forms. This allows you to spend less time on the busy work of applying for admission, and more time on what's really important: college research, visits, essay writing, and senior year coursework.

·      Counselor Information: Luann Curry, CurryL@hasd.us.  Please do not use @myhalifax.us email address.

·      Use your own personal email address, not your school email address.

 Step 2: Request a Transcript

Request to have your transcript sent to schools where you have applied.  To request your transcript, click here Request a Transcript Form or go to the School Counseling webpage.

·      These transcripts should be requested as official transcripts.

·      Some schools will have you self-report your grades/scores.  For this you should request an unofficial transcript for yourself.

 Step 3: Request a Recommendation

Request a letter of recommendation – consider teachers who can speak to who you are currently, and who are most relevant to your choice of study.  To request a recommendation, you need to go to our webpage and click here Recommendation Request Form or go to the School Counseling webpage. Please allow the recommender at least ten (10) school days to complete their recommendation.
